Since 1997, our practice has been providing excellent care in a patient focused environment. Our staff is highly experienced and seeks to treat patients as we would want to be treated.
Communication is the key. By listening carefully and taking the time necessary at appointments to answer your questions, we can provide better care and you’ll feel more comfortable. Dr. Firdos Sheikh invests her time towards building strong relationships between her patients not only as their doctor but also maintains a friendly relationship even after treatment has been completed.
When you call, patient appointments are set as quickly as possible and we make every effort to keep wait times in our office at a minimum. As a courtesy, we ask that you give our office 24 hours advance notice if for some reason you need to cancel your appointment.
What to bring to your first appointment:
* Your insurance card
* Physician referral forms if required by insurance
* A list of current prescription or over-the-counter medications you are taking, including dose and frequency
* Pertinent information about your medical and surgical history
* Any pertinent records, films or test results you may have
* Any patient forms you might have filled out in advance (see below)

Patient Medical Information
After Hours Calls
Dr. Sheikh’s concern for her patients does not stop when she leaves the office. If there is an emergency or serious medical problem requiring immedicate care after hours, the patient or designee should call the office number. The answering service will take the caller’s name and telephone number and contact the doctor or the physician-on-call who will respond as soon as possible.
Prescription renewals are authorized during office hours. The patient is asked to have the pharmacist’s name and telephone number handy when calling Dr. Sheikh’s office for a renewal.
Payment & Insurance Information
Dr. Firdos Sheikh, M.D., is a provider for Medicare and most major insurance plans. We provide insurance billing. Anything not covered by insurance will be your responsibility. We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.
